Participant Agreement

1. Program Consent & Acknowledgment of Policies

I understand that I am enrolling in a court-ordered educational program offered by GraceRoot Institute, operated by Academic Research Solutions, Inc. dba Dareu2bu Counseling Services. I have read, or have had the opportunity to read, the following program policies prior to accessing course content:

• Grievance Policy (available at graceroot.institute/grievance-policy)
• Confidentiality Policy (available at graceroot.institute/confidentiality-policy)

I consent to participate in this court-ordered educational program under the terms described in these policies.

2. Acknowledgment of Completion Criteria

I understand that to receive a completion certificate I must:

• Complete all modules in sequential order
• Pass all required module quizzes at a minimum score of 70%
• Engage meaningfully with all instructional content

I understand that my certificate will be issued automatically upon meeting these requirements, and that it is my responsibility to meet any court-imposed completion deadlines. GraceRoot does not grant extensions on behalf of courts.

3. Consent to Share Progress with Referring Representative

I understand that GraceRoot does not automatically share my progress or completion status with courts or supervising officers. I may use the "Send to My Representative" feature on my dashboard to voluntarily send completion documentation to my attorney, probation officer, parole officer, case manager, or other court representative.

By using that feature, I am explicitly consenting to the sharing of my enrollment and completion information with the specific recipient I designate at that time.

4. Academic Honesty

I certify that all work submitted through this program is my own. I will not:

• Share quiz questions, answers, or answer keys with other participants
• Use AI tools or other automated means to answer quiz questions on my behalf
• Allow another person to access my account or complete coursework on my behalf
• Provide false information on any program document

I understand that confirmed academic dishonesty will result in immediate removal from the program, revocation of any certificate issued, and potential notification of the referring court. No refund will be issued for removal due to academic dishonesty.

5. Certificate Conditions

I understand that my completion certificate:

• Is issued only upon meeting all completion requirements
• Is identified by a unique CO-xxxx certificate number
• May be verified by contacting GraceRoot directly
• Will be revoked if academic dishonesty or fraudulent completion is confirmed
• Is retained in GraceRoot's records for a minimum of 7 years

6. Grievance Policy Acknowledgment

I understand that I have the right to file a formal grievance if I believe I have been treated unfairly or that my rights have been violated. I understand that filing a grievance will not affect my program standing, completion status, or certificate eligibility, and that I will not face retaliation for using the grievance process.

7. Confidentiality Policy Acknowledgment

I understand how GraceRoot collects, stores, and protects my personal information and program records, as described in the Confidentiality Policy. I understand:

• My records are stored securely and access is limited to authorized administrators
• My records will not be shared with courts or supervising officers except with my explicit consent or as required by law
• I have the right to review my own records and request corrections
• Records are retained for a minimum of 7 years

This agreement constitutes a legally binding electronic contract. By signing below with your full legal name and checking all acknowledgment boxes, you agree to all terms stated above. Your signature, IP address, device information, and timestamp are recorded as part of this agreement.
