Authorization for Release of Records

Use this form to authorize GraceRoot Institute to release documentation of your enrollment, attendance, and completion of a court-ordered program to a court, probation department, attorney, or other referring authority. The authorization below follows the release-of-information standards of the HIPAA Privacy Rule (45 CFR §164.508). You may complete it online or print it, sign it, and return it.
